Option Care Health is seeking a Clinical Transition Specialist to educate patients, families and facility staff about home care options. The role focuses on placing patients into appropriate home or alternate site care and building partnerships with hospitals and referral sources. Responsibilities include coordinating pre-admission data, working with intake and discharge planners, and supporting sales goals with Account Managers.
