Care Coordinator

Liberty Resources is seeking a Full-Time Care Coordinator with our Assessment Center for Early Support (ACES) program located in Potter County, TX. ACES is aimed at supporting youth experiencing school-related challenges or peer issues. This program is designed to provide early intervention and community coordination to help keep children out of the juvenile justice system. Provides Care Coordination that supports youth and family success. Completestimlyandaccuratedocumentation including but not limited tointake,assessment,referrals,and follow-up interactionsin the designated EHR, and uploadsadditionalpaperworkas necessaryinto theEHR. Maintains confidentiality and adheres to ethical and legal standards Shares on-call responsibilities with the Program Director. Makescommunity referrals and recommendations based on the identified needs of the youth and family to prevent the youth from entering or progressing further into the juvenile justice system. Assiststhe youth and family in accessingcommunityresources. Conductsfollow-up contactsas necessaryto support youth and family success. Care Coordinator Qualifications: Bachelor'sdegreeand two (2) yearsofexperienceworking withmulti-need youth and familiesormaster's in social workor related clinical degree and one (1) year post-graduate experience. Prior related experience may be substituted for one (1) year post-graduate requirement. A general knowledgeof andpractical experience working with families and their needs is preferred. Must have a valid State of Texas driver’s license and access to reliable transportation.
