They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed. Territory Details: San Francisco County, San Mateo County, and Santa Clara CountyRequirements: One year of Hospice experience.\n Position Urgency:\n Normal\n Shifts:\n Days: Sunday - Thursday Hours: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient's availability.)\n OR\n Days: Tuesday - Saturday Hours: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient's availability.)
