Hospice RNJob Title: Hospice RN Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Days: Sunday - Thursday Hours per Shift: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient's needs) Experience: 1 year of Hospice experience License: Valid nursing license Certifications: Current BLS certification Must-Have: One year of Hospice experience Description: The Hospice Nurse admits patients to service. They assess the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. The nurse develops an individualized plan of care based on their assessment. They document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team. The nurse participates in the hospice program's quality assessment performance improvement program. They conduct follow-up visits as needed. The position covers a specified geographical area.
