Senior Care is seeking a compassionate, organized, and self-motivated LPN, RN, or Licensed Social Worker to provide Targeted Case Management (TCM) services to Medicaid beneficiaries.

This role is ideal for someone who enjoys independence, flexibility, and meaningful one-on-one work helping individuals access the care, services, and resources they need.

???? Position Responsibilities

As a Targeted Case Manager, you will assist Medicaid beneficiaries in gaining and coordinating access to medical, social, educational, and other services appropriate to their individual needs.

Responsibilities include:

• Completing assessments to identify individual needs, strengths, resources, and goals
• Developing and maintaining individualized plans of care
• Coordinating access to medical, social, educational, and community-based services
• Making appropriate referrals to providers and community resources
• Communicating with beneficiaries, families, caregivers, healthcare providers, and service agencies
• Monitoring services and completing required follow-up
• Identifying changes in needs and updating plans as appropriate
• Maintaining accurate and timely case management documentation
• Helping beneficiaries overcome barriers to accessing needed services

✔ Who We’re Looking Fo r

Candidates must be licensed in Arkansas as one of the following:

• LPN
• RN
• Licensed Social Worker

Additional qualifications:

• Strong communication and organizational skills
• Self-motivated and comfortable working independently
• Ability to effectively manage a caseload and required documentation
• Reliable transportation
• Case management, Medicaid, home care, long-term care, or community-based experience is a plus
