Job Description
The Referral Partner Program Specialist provides referrals, follow-up support, and resource navigation to low-income clients across an 18-county service area. This position works closely with Community Resource Navigators to assist individuals with accessing social services, public benefits, financial assistance, utility support, and other community resources. The role requires strong documentation, case management, customer service, and communication skills while ensuring compliance with HIPAA, state requirements, and program guidelines. The ideal candidate is compassionate, organized, detail-oriented, and comfortable working with diverse populations.

Responsibilities:
• Conduct client intake assessments, provide referrals, complete follow-up services, and maintain accurate case notes and documentation
• Assist clients with gathering required documentation and completing applications for public benefits, financial assistance, utility assistance, and other community programs
• Enter timely and accurate client information into required reporting platforms and maintain compliance with state forms, reporting requirements, and program timelines
• Build and maintain relationships with community-based organizations, social service agencies, referral partners, and clients while increasing awareness of available programs and resources
• Protect confidential client information and ensure HIPAA compliance while supporting departmental goals, training initiatives, reporting requirements, and disaster recovery efforts as needed

Qualifications and Requirements:
• Bachelor’s degree in Social Work, Social Sciences, Marketing, Business, or a related field preferred; or at least 2 years of related casework or case management experience
• 2+ years of experience determining eligibility for city, county, or state benefits preferred
• Strong knowledge of HIPAA requirements, case documentation, public benefits applications, and confidential information handling
• Proficiency in Microsoft Office, including Word, Excel, PowerPoint, and Outlook, with the ability to learn new software systems
• Excellent written and verbal communication, customer service, organization, time management, problem-solving, and multitasking skills; bilingual English/Spanish preferred

The base pay range for this position is $21.00 an hour, excluding benefits, bonuses, or other compensation. Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity. Please note, hiring at the top of the range is uncommon to allow room for future growth.
