Registered Nurse - Hospice

Job Description

Job Summary:
Provide specialized hospice nursing care to patients in a home care setting, ensuring comfort and quality of life for terminally ill patients. Manage caseloads effectively while adhering to hospice protocols and standards.

Responsibilities:

Assess patient needs and develop individualized care plans aligned with hospice guidelines.
Deliver compassionate nursing care in patients' homes, including symptom management and education.
Collaborate with interdisciplinary teams to coordinate ongoing care and improve patient outcomes.
Document all clinical interactions accurately within the McKesson/Netsmart EMR system.
Conduct routine visits, monitor patient progress, and adjust care plans as necessary.
Ensure compliance with all hospice and healthcare regulations and standards.

Qualifications:

Hold an active RN license with recent hospice case management experience.
Demonstrate experience providing hospice care in a home setting.
Have prior experience with rural travel to serve patients in diverse locations.
Be proficient with McKesson/Netsmart EMR systems.
Possess excellent communication and compassionate patient care skills.
Preferably have no RTO (Request for Time Off) restrictions during the contract period.
