Our model blends embedded hospital support with advanced data tools to reduce length of stay (LOS), alleviate weekend discharge bottlenecks, and transform how care transitions happen.

We are seeking an experienced Case Manager (LCSWs, MSWs, & RNs) to manage complex discharges for our partner hospital in the Towsan, MD area. This role is ideal for a motivated, outcomes-driven professional who thrives in fast-paced environments and wants the flexibility, creativity, and autonomy to improve how patient discharges are handled.

This position allows direct access and communication with leadership and additional supportive resources.

What You'll Do

Facilitate safe and successful discharges for complex patients by navigating barriers and driving innovative solutions across hospital teams and post-acute care settings within SilverStay's preferred provider network, including SNFs, LTACHs, acute rehab, home health, and community resources.

Use creativity and resourcefulness to solve discharge challenges without the constraints of hospital bureaucracy.

Participate in hospital team rounds, care team huddles, and engage with patients and families to ensure seamless communication across all stakeholders.

Present reports to hospital leadership on progress, scalability efforts, and results, proposing and executing adjustments as needed.

Build and refine scalable workflows and processes that increase efficiency and outcomes. Partner with SilverStay leadership and hospital teams to track key metrics (LOS, readmission rates, discharge success) and continuously improve results.

Leverage technology and data-driven insights to transform how complex cases are managed.

Who You Are

Must be a licensed RN, LMSW, or LCSW.

Must have 3+ years' on-site hospital, adult inpatient case management, discharge planning, or transitional care experience. This is a critical requirement.

Skilled at navigating post-acute settings (SNF, LTACH, acute rehab, group health).

Experience using EMR referral platforms such as Telligen, Careport/WellSky, EPIC, and Cerner.

A patient and family-centered communicator with empathy, professionalism, and persistence.

Ambitious, entrepreneurial driven, innovative, dedicated and diligent work ethic.

Thrives in high-responsibility, high-ambiguity environments where adaptability and creativity are key.

Tech-savvy and excited about the role of innovation in healthcare.

Must be BLS certified.

Compensation & Benefits

Competitive Salary: $100,000 - $130,000; commensurate with experience and potential impact.

Performance-Bonus: Bonus structure that can add 10% to 40% of base salary, tied directly to outcomes and results. Paid out quarterly.
