Care Team Manager - Behavioral Health Location: Texas | FLSA Status: Exempt | Job Status: Full Time | Work Model: Hybrid | Work Schedule: Monday–Friday with occasional evenings and/or weekends | Vehicle Required: Yes | Amount of Travel: Occasional domestic travel in Texas | Positions Supervised: Behavioral Health Community Guides | Reports To: Assistant Director of Care Team Operations Responsibilities 90‑95% leadership responsibilities: Understand Reema's program, goals, expectations, and metrics for Community Guides individually and as a team; meet accountability and communicate expectations aligned with values. Champion change management and lead hybrid Care Teams through ambiguity and innovation. Establish structures and processes that cultivate communication and accountability, including: Facilitating regular 1:1 meetings with Guides; setting and monitoring individual Guide goals around metrics, Guide activity, and/or member interactions. Leading engaging team meetings, multidisciplinary team meetings, case reviews, Community Volunteering, and Team Building Events. Review Community Guide performance to deliver consistent feedback and recognition. Partner with clinicians for service alignment, manage multidisciplinary workflows and communication, ensuring high‑quality service delivery. Facilitate weekly check‑ins that include staffing and metrics updates, reporting, and setting monthly Care Team goals with the Director of Clinical Care Team Operations. Supervise guides within assigned teams, researching resources for members. Champion partnered outreach and support other Community Guides in community outreach. Participate in hiring process as lead partner in Care Team interviews. Facilitate employee onboarding and ongoing staff training and development. Ensure the team maintains PHI confidentiality and follows HIPAA and software‑security policies. Initiate and maintain relationships with local community partners across the mental‑health continuum of care. 5‑10% direct‑service responsibilities with members: Use cold outreach to establish trusting relations with an empathetic, strength‑based, trauma‑informed approach. Engage via text, calls, home visits, and other face‑to‑face communication to meet member needs. Know community resources; refer, follow‑up, attend appointments as necessary. Be present in crisis and everyday life challenges. Encourage members toward independence and autonomy. Education, Experience, and Skills Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field (mandatory). Three years of people‑management/supervision experience in health‑care, mental‑health, community‑based or social‑work settings. Five years of direct experience with the local population served and knowledge of the community. Proficiencies in critical thinking, strategic thinking, time management, and prioritization. Knowledge of crisis planning, case management, and behavioral‑health. Experience working with individuals from diverse ethnic backgrounds, serious persistent mental illness, and substance‑use needs. Experience on multidisciplinary teams alongside clinical and non‑clinical staff. Experience with adolescents and families. Experience with hard‑to‑reach populations (e.g., homeless). Propensity to navigate ambiguity and creative problem‑solve. Propensity to build and maintain relationships. Propensity to initiate conversation, create connections, develop, adapt, and execute outreach plans. Propensity for fast‑paced environments; start‑up experience is a plus. Fluency in Google Workspace applications (mandatory). Comfortable with web‑based platforms/apps across mobile and desktop; self‑troubleshoot technical issues. Qualifications & Position Requirements Must be 18 years of age or older. Must have reliable access to transportation, valid driver’s license, vehicle insurance, satisfactory driving record. Must pass pre‑employment drug screening and background check. Must comply with federal, state, and local confidentiality obligations (HIPAA). Must use technology‑based tools daily.
