For more information regarding our work and focus visit Job Summary As a Wrap Care Coordinator at CK Family Services, you will facilitate the Wrap-around planning process to the family. The following list identifies skills necessary for effective Wraparound Care Coordination. This list of skills should be conceptualized as a tool kit for Care Coordinators. The list is divided into the four phases of Wraparound: Phase 1: Engagement and team preparation, Phase 2: Planning, Phase 3: Implementation and Phase 4: Transition. You will also be trained in family-oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and case management services to clients of the agency per state specified standards and requirements. General Responsibilities Provide targeted (intensive) Case Management services to families and document within 2 business days. Facilitate the Wraparound planning process and all tasks required within each phase, billed by Intensive Case Management. For all details please refer to the Wrap Facilitator checklist. Actively use the Engagement and team Preparation skills as identified by the Wraparound program throughout all phases. Engage the family and various team members throughout the Wrap Around process. Communicate and operationalize the four key elements: Determined by Family, Strengths Perspective, Driven by underlying needs, Supported by an Effective Team. Facilitate and plan Child and Family Team Meetings, and demonstrate the ability to handle and resolve conflict. Obtain regular updates from the Legal Caregivers regarding the child’s permanency plan or the family’s vision and progress towards the plan. Effectively oversee the plan of care and monitor progress by following up with each family and team member between CFTMs. Schedule and facilitate all Child and Family Team Meetings (CFTM) every 30 days, and Emergency CFTMs within 72 hours of a crisis. Develop a Crisis Safety plan at the initial meeting and continue to monitor and update as needed. Submit each Plan of Care within five business days after the meeting. Monitor daily implementation of the plan of care and track progress weekly. Develop a transition Plan of Care when the child and family move towards the end of the Wrap process. Complete all case records accurately and timely within two business days. Document services respecting that all clinical notations are legal documents, accurate and truthful. Participate in Wrap around supervision support. Immediately contact your supervisor and file a report with DFPS if you suspect a consumer has been a victim of abuse, neglect or exploitation. Immediately contact your supervisor if you have been called to testify or be involved in legal proceedings. Maintain client confidentiality, HIPAA laws, HIPP regulations and professional certification or licensure requirements. Present yourself in a professional manner, representing CK Family Services. Be aware of formal services within the agency and community. Serve on task forces or work groups as requested. Provide consultation to vendor agencies as required. Obey all city, state and federal laws, rules and regulations. Perform duties without violating ethical standards, state or federal codes or agency requirements. Be familiar with the Policies and Procedures Manual, Employee Handbook and Employee Expectations. Report critical incidents, including suspected child abuse, neglect or exploitation, immediately to the Abuse and Neglect Hotline and Director. Ensure compliance with all rules and regulations of State and Federal regulatory agencies DSHS, licensure rules and suspected fraudulent practices. Capture all changes and modifications to the plan of care and send them out within five business days. Participate in clinical and agency administrative meetings as required. Provide transportation as required. Respond to phone calls, messages, emails within one business day, open and process mail/email daily. Answer phones, collect messages and respond to requests promptly and accurately. Input data into computer accurately and timely. Represent CK Family Services in the community by attending scheduled meetings. Maintain confidentiality as required by government law and maintain compliance with all CK Family Services policies, procedures and state and federal laws. Perform all other duties and tasks as requested. Qualifications Minimum bachelor’s degree in rehab counseling, psychology, social work or a related field as specified by the Texas Administrative Code for QMHP qualifications. General understanding of adult and child mental health disorders, treatment modalities and documentation of treatment provided. Knowledge of confidentiality and ethical guidelines for practice. Excellent organizational and follow‑up skills. Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people while maintaining confidentiality. Training in working with individuals with Severe and Persistent Mental Illness, Children with Severe Emotional Disturbances and delivery of services based on curriculum. Ability to work with small details, visual inspection and maintain focus on data and figures. Ability to receive verbal instructions accurately in some background noise. Ability to motivate people and develop supervisory skills. On‑call availability as necessary with immediate cellular phone response. Ability to handle multiple tasks; self‑starter; energetic; detail oriented; excellent organizational skills. Good character, personal attributes and mannerisms to model behavior for clients and families. Ability to work effectively under stress and remain flexible in changing situations. Ability to concentrate on moderate to fine detail with occasional interruptions. Ability to understand and relate to multiple ideas and concepts, and to remember multiple verbal and written tasks or assignments over several days or months. Proficient knowledge of computer software: word processing, email, spreadsheets, database. Ability to participate in training and facilitate training as required. Physical demands: up to 100 pounds of force occasionally, 20 pounds frequently; ability to step, grasp, bend, lift, kneel and move; sufficient good health to perform role duties.
