Title
Case Management - Acute Care

Schedule
Every other weekend required.

Assignment Details
Contract length: 13 weeks
Guaranteed hours: 40 hours per week

Requirements
- Valid state license required; compact license must be held at submission
- Minimum of 2 years of acute care hospital case management experience required, current within the last year
- Experience with electronic medical records system: Epic
- References: 2 references covering 1 year within the last 3 years required

Roles & Responsibilities
- Facilitate patient care coordination and discharge planning for diverse patient populations.
- Conduct utilization review and ensure proper care protocols are followed.
- Process prior authorizations and handle appeals and denials as needed.
- Collaborate with interdisciplinary teams to optimize patient outcomes and ensure compliance with advance directives and protective services.

Compliance / Notes
- Must commit to working 3 of these 5 holidays if required: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.
- Include all requested RTO at submittal.
- Free parking provided during orientation.
