Day shift, 5x8 hours (08:00 - 16:30). Required to work every other weekend.

Job Description

We are seeking a qualified Case Management Registered Nurse with acute care experience to join our team. This role involves providing essential case management services, including discharge planning, care coordination, and patient advocacy. The candidate must be able to float between like units as needed, based on departmental requirements.

Responsibilities

• Conduct assessments and develop care plans according to patient needs.
• Coordinate with interdisciplinary teams to ensure comprehensive care.
• Manage admissions and discharges while ensuring smooth transitions for patients.
• Handle appeals and denials, ensuring compliance with protocols.
• Provide guidance on benefits eligibility and prior authorizations.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care experience required.
• Must possess a valid RN license in good standing.
• Familiarity with Epic charting system is required.
• Experience with HMOs, IPAs, advanced directives, and protective services process is preferred.
