Manny Cantor Center at Educational Alliance brings together a wide range of high quality, multi-generational programs and services to serve the Lower East Side community as well as other parts of NYC. We are seeking a part-time Case Manager to work with our Community Programs and Social Services team. The Case Manager will provide case assessment, referral services and case management to a diverse community population with the aim of enhancing their well-being and socioeconomic mobility.
RESPONSIBILITIES
• Maintain a caseload of 10-15 clients, conducting intake assessments for public benefits and socioeconomic needs. Provide support by assisting with benefits applications, making referrals, and connecting clients to appropriate resources.
• Participate in regularly scheduled case conferences, staff meetings, and training sessions, while also providing assistance for cross-departmental events.
• Participate in community outreach events to promote the services offered by Community Programs and Social Services and establish networking connections.
• Maintain up-to-date case records and program statistics in Trax Solution database.
• Accompany clients to appointments for benefits and entitlement meetings when deemed appropriate.
• Other duties as assigned
REQUIREMENTS
At least one year of experience working within a community-based organization
Must be available to work Saturday shifts
High School Diploma or equivalency
Knowledge of available community resources, such as public benefits, housing assistance, healthcare services, employment support, and educational programs, and the ability to connect clients with appropriate resources.
Excellent communication skills, both written and verbal.
Proficiency in using MS Office applications such as Word, Excel, and PowerPoint.
Mandarin speaker preferred
