Key Responsibilities\n

• Develop and implement comprehensive discharge plans, ensuring continuity of care and smooth transitions for patients exiting care.

• Establish and maintain partnerships with community organizations to facilitate access to necessary post-discharge services.

• Document all patient interactions and treatment plans accurately and timely in compliance with legal and organizational guidelines.

• Engage in crisis intervention when necessary, providing immediate emotional support to individuals and families in distress.

• Collaborate with medical teams in case conferences to optimize treatment strategies and improve patient satisfaction.

Qualifications\n
