HealthPoint is a community-based, community-supported and community-governed network of non-profit health centers dedicated to providing expert, high-quality care to all who need it, regardless of circumstances. Founded in 1971, we believe that the quality of your health care should not depend on how much money you make, what language you speak or what your health is, because everyone deserves great care .

Position Summary:

The HealthPoint non-core Family Medicine Residency Faculty Member performing Adult Inpatient Medicine is responsible for educating and supervising residents while competently practicing adult inpatient and transitional care medicine. The HealthPoint non-core Family Medicine Residency Faculty Member performing Adult Inpatient Medicine is responsible for educating and supervising residents while practicing adult inpatient and transitional care medicine in accordance with standards of competency defined by the residency program and hospital. Acknowledging that Hospital Medicine is its own medical specialty, as recognized by the American Board of Hospital Medicine and Society of Hospital Medicine, Faculty will be expected to have appropriate training and expertise to provide evidence-based, high-quality inpatient and transitional care medicine. Clinical care is to be performed in alignment with the Residency’s mission of addressing healthcare disparities. These requirements are separate from, and in addition to any requirements the Faculty Member has with relation to any other position held within HealthPoint.

Clinical practice is expected to be comprehensive, high quality, evidence-based and to demonstrate cultural humility. It is dedicated to serving the underserved and vulnerable, where care is complicated by limited resources and high rates of psychiatric and socioeconomic disease. Care is to be provided by the residency teaching service in collaboration with interdisciplinary teams, community partners and hospitals, though the Faculty Member remains ultimately responsible for the patient’s outcome.

Educational responsibilities require the Faculty Member to work closely with residents as both a teacher and mentor. The residents shall receive high-quality training in inpatient and transitional care medicine, with the goal of fostering growth and competency in these realms. Teaching and supervision are to be commensurate with the resident’s learning level and are to be aligned with the Residency’s mission.

Compensation is dependent on skills and experience.

Inpatient Essential Duties and Responsibilities:

Clinical Care and Patient Management

  • Provide comprehensive medical services in hospital care, working independently and interdependently with other hospital specialists and sub-specialists.
  • Provide inpatient care for HealthPoint patients and patients without a PCP. This care may take place in the Emergency Department, Medical-Surgical Floor, PCU, ICU or Behavioral Health Unit.
  • Receive and manage patients for hospital admission and coordinate all aspects of inpatient care in collaboration with medical staff, including:
    • Admissions
    • Appropriate placement within the hospital
    • Consultations with sub-specialists
    • Timely and proactive discharge planning
    • Coordination of care with primary care physicians
    • Discharge including ensuring appropriate follow-up is arranged
  • Function as the Medicine consultant to all areas of the hospital, including timely consultations to the Emergency Department, surgeons, behavioral health unit and other Medical Staff members on request.
  • Perform a History and Physical Exam and develop an Assessment and Plan for every patient. The History and Physical Exam are to be performed face-to-face with the patient.
  • Provide real-time clinical decision support during emergent situations, code blue responses, and rapid response team activations.
  • Responsible for accurate, timely, and complete documentation of inpatient records. Documentation may be an attestation of a resident Admission, Progress, Event or Cross-Cover Note, as appropriate.
  • Provide patients and family members with counseling and education regarding diagnosis, treatment, and need for follow up as appropriate during each encounter. Verify and document that the patient understands diagnosis, treatment, and follow up recommendations.
  • Ensure continuity of care during transitions between shifts, weekends, and holiday coverage periods.
  • Serve as the attending physician of record, assuming full responsibility for all clinical decisions and oversight of patient care.

Resident Education and Supervision

  • Provide additional supervision, feedback, and documentation review for struggling learners, recognizing that these residents require increased time, oversight, and faculty expertise to ensure safe patient care and effective educational support in the inpatient setting.
  • Perform direct and/or indirect supervision of all resident care, with a level of supervision to be determined by the resident’s level of training, Individualized Learning Plans and other factors, and as directed by the Program Director.
  • Engage in one-on-one discussions with the residents regarding the care of every patient seen in the hospital.
  • Review and addend resident charts from the inpatient setting and critique the resident’s case presentation and medical record documentation.
  • Teach residents and medical students in adult inpatient and transitional care medicine by a combination of direct and indirect supervision, rounds discussions, Faculty Member documentation, didactics and role‑modeling if excellent medical practice.
  • When appropriate, conduct bedside teaching during patient encounters, demonstrating clinical reasoning, differential diagnosis development, and treatment plan formulation.
  • Teach and observe physical examination techniques, ensuring residents develop proficiency in comprehensive patient assessment.
  • Model patient communication skills including difficult conversations, informed consent discussions, and family meetings.
  • Demonstrate integration of evidence‑based medicine into clinical decision‑making processes.
  • Provide instruction in clinical documentation.
  • Teach residents to recognize and respond appropriately to clinical deterioration and medical emergencies.
  • Participate in medical education and evaluation of residents and medical students using the core competencies set forth by the ACGME such as fund of knowledge, diagnostic processes, treatment options and follow up care.
  • Provide real‑time formative feedback during clinical encounters, emphasizing specific, actionable, and timely input.
  • Serve as mentor for residents, providing guidance on career planning and professional development.
  • Support residents in the development of leadership skills, teaching abilities, and scholarly pursuits.
  • Provide letters of recommendation and career references for residents pursuing fellowship training or employment opportunities.

Administrative and Professional Work

  • Maintain essential credentials for practice, including licensure, certification, professional society memberships, and continuing medical education (CME) for adult inpatient clinical practice.
  • Maintaining good attendance, being punctual, and working the full scheduled shift.
  • Demonstrate respectful, professional, and appropriate behavior that supports a team‑oriented work environment.
  • Demonstrate exemplary professional behavior, ethical decision‑making, and interprofessional collaboration.
  • Teach residents about cultural competency, health disparities, and social determinants of health.
  • Demonstrate work‑life balance and physician wellness practices.
  • Address unprofessional behavior immediately through direct feedback and corrective action plans when necessary. Role‑model professional behavior in hospital settings by addressing unprofessional conduct between staff, between staff and patients, or other interprofessional conflicts. Guide residents in observing and learning how to provide respectful feedback and navigate these complex situations effectively.
  • Participate in hospital daily safety huddles and when necessary, in hospital‑wide patient safety initiatives.
