Physical Therapist Full Time Professional Washington, DC, US 8 days ago Requisition ID: 1029 Salary Range: $115,000.00 To $125,000.00 Annually Role: Physical Therapist Organization: Edenbridge PACE at Skyland Location: Washington, DC SE Status: Full-time Mission: To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives JOB SUMMARY The Physical Therapist Position offers a unique opportunity to help build the first PACE (Program of All-Inclusive Care for the Elderly) program in the District of Columbia within the framework of a supportive, innovative, and expert leadership team and company. The Physical Therapist Position is responsible for delivery of appropriate physical therapy evaluation and treatment to Edenbridge PACE at Skyland Town Center participants on acute, restorative or maintenance levels as needed. This position reports to the Edenbridge PACE at Skyland Town Center’s Center Manager. ESSENTIAL DUTIES AND RESPONSIBILITIES The following essential functions of this position are performed personally, in cooperation with your supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned. Serves as a required member of the Interdisciplinary Team and attends IDT meetings daily. Performs timely initial and annual assessments for all participants and corresponding plans of care. Reviewing patients’ medical histories Diagnosing patients by observing their movements and listening to their concerns Developing individualized treatment plans for patients Outlining clear goals for patients and the expected outcomes of the plan Examining participants with impairments, functional limitations and disabilities in order to determine a diagnosis, prognosis and course of intervention while utilizing tests and measurements consistent with the practice of physical therapy Use therapeutic interventions within the scope of physical therapy practice to prevent or manage injury, impairment, functional limitation, and disability, including the promotion and maintenance of health, wellness, fitness and quality of life Recording patients’ progress and modifying the plan of care if needed Performs semi-annual reassessments and unscheduled change of condition assessments for all participants receiving physical therapy services as well as corresponding plans of care. Participates in home safety evaluations as indicated for potential enrollees into the program Responsible for updating care plans and measuring the outcomes for physical therapy goals. Assists in the delegation of therapy plans for specific participants to Physical Therapy Assistants according to complexity of participant’s needs and nature of required physical therapy procedures. Trains family members, caregivers and other staff members in methods to enhance mobility, safety and functional independence of participants. Assists in the ordering of durable medical equipment for participants. Other duties as requested/assigned QUALIFICATIONS A graduate of a physical therapy program accredited by the American Physical Therapy Association is required. Must hold an active license with the DC Department of Health Must submit to and pass a pre-employment drug/alcohol screening and criminal background check. Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance. Must have working knowledge of computers and Microsoft Office applications.
