About the Role
**This position is based in Wilson, NC but will also support Fayetteville, NC virtually**
The ideal candidate thrives in a fast paced, evolving climate while meeting the needs (Healthcare, Housing, Food Support) of some of the communities most vulnerable members. This role requires a candidate that is highly organized, excellent time management skills, and strong social boundaries. The candidate must be comfortable managing a high volume of outbound and inbound member calls on a daily basis . Must demonstrate strong assertive engagement skills, with the ability to effectively connect with hard-to-reach members and build rapport with individuals who have complex needs. Display experience working in a performance-driven environment with established weekly and monthly productivity standards, quality metrics, and performance expectations.
The Care Management Extender is an integral part of the Tailored Care Management team approach to integrated care for behavioral health and medical care by supporting the members through collaborative efforts that improve member outcomes with activities such as linking members to medication management, coordinating physical and behavioral health care needs, educating members, building trust between members and their practitioners, supporting and connecting members to community recovery programs, and enhancing communication and continuity of care.
- Monitor members (in person and by telephone) for changes in clinical symptoms and treatment side effects or complications;
- Evaluate findings and assist members/families in developing an Individualized Service Plan (ISP) with appropriate goals;
- Support and closely coordinate whole-person health care with the member's primary care provider and other treating health providers.
- Provide or facilitate resources internally or in the community for members/families, and make referrals and provide follow-up as appropriate (e.g., social services such as housing assistance, vocational rehabilitation, substance use treatment);
- Ensure all treating providers involved in the member's care are up to date on the latest test results and information.

Competitive hourly pay aligned with your role, experience, and contribution, with opportunities for growth over time.
- Compensation: $17.36 - $22.29
Placement within the range is based on licensure level, education, and experience.
What You Can Expect
- Work in a fast-paced environment supporting members with healthcare, housing, food assistance, and other essential needs.
- Manage a high volume of inbound and outbound calls while building trust and engaging with members who have complex needs.
- Coordinate care by connecting members with healthcare providers, community resources, and support programs.
- Monitor member progress, assist with care planning, and help ensure communication between members, families, and treatment providers.
Responsibilities
- Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
- Connect members and families to healthcare services, housing assistance, food resources, and community programs.
- Assist in developing and updating individualized care plans to support members' health and wellness goals.
- Coordinate communication among healthcare providers, members, and families to ensure continuity of care and follow-up on referrals.
Qualifications
- A high school diploma or equivalent;
- Must be 18 years or older.
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist
or
- A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual
or
- Have two years of paid experience performing the types of functions described in the “Extender Functions” section, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
- Upon hire, completion of general onboarding training;
- Within 60 days of hire, completion of NC DHHS identified training requirements for Tailored Care Management
- No driving infractions in the last 5 years
