Job Title Behavioral Health Clinician (LSW/LPC) - SMI Care Management Department Harding | Harding Social Work Scope of Position OSU Harding Hospital and East Hospital–Talbot Hall deliver the most comprehensive adult behavioral healthcare services in central Ohio, offering the region’s broadest range of treatment options and a highly experienced multidisciplinary team. Services span psychiatric emergency care, behavioral health urgent care, inpatient and outpatient treatment, intensive outpatient (IOP) and partial hospitalization programs, as well as specialized, evidence‑based care for mood and anxiety disorders, obsessive‑compulsive disorder, psychosis, trauma‑and stress‑related disorders, sexual dysfunction, and neurocognitive conditions. Talbot Hall provides the area’s most extensive addiction services, including detoxification, medication‑assisted treatment, traumatic brain injury‑specific programming, and a full continuum of outpatient and continuing‑care groups. Care is delivered through an integrated, patient‑centered model that emphasizes safety, continuity, and individualized treatment planning. Position Summary The SMI Behavioral Health Care Management Clinician‑2 provides intensive office‑and community‑based care management for adults with serious mental illness (SMI) who are admitted to or discharged from Harding Hospital’s inpatient psychiatric units. These patients often present with severe and complex conditions, including treatment‑resistant psychosis, major mood disorders, suicidality, co‑occurring substance use, and other high‑risk symptoms. Many are discharged on clozapine, for which OSU Harding is the highest‑volume prescriber in Ohio. This role ensures comprehensive medication oversight, including REMS‑required laboratory monitoring, coordination of transitions of care to OSU EPICENTER or community mental health providers, and development of care plans to reduce emergency department utilization and prevent readmissions. In partnership with the OSU EPICENTER BH team, the SMI Behavioral Health Care Management Clinician 2 supports an enhanced care model focused on continuity, rapid linkage to outpatient services, and sustained engagement post‑discharge. Responsibilities Coordinate care and link patients to community resources. Support continuity of care, encourage compliance, conduct assessments and develop treatment plans. Initiate short‑term psychosocial interventions, discharge planning, and assist patients in overcoming barriers to social determinants of health (transportation, finances, insurance, housing, food insecurity, etc.). Engage patients before, during, and after hospitalization to promote treatment adherence and address social determinants of health. Collaborate with psychiatrists, nurse practitioners, nurses, social workers, and other mental health and substance use disorder professionals to achieve patient care outcomes, reduce unnecessary hospitalizations or readmissions, avoid relapse, and facilitate linkage to social determinants of health. Maintain knowledge of behavioral health and substance use disorder best practices, regulatory requirements, and community resources. Minimum Qualifications Master’s Degree in Social Work or Counseling or education accepted by the State of Ohio for licensure. Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), or Licensed Chemical Dependency Counselor II or III (LCDC) required. LPC or LSW preferred. 0 years of relevant experience required. 0‑2 years of relevant experience preferred.
