Home Health Aide/Hospice CNAThe Home Health Aide/Hospice CNA administers personal care, homemaking, and respite care. Prepares and issues equipment and educates patient and families on use.Job RequirementsHigh school graduate preferred.Completion of Certified Aide Course.BLS certification.One year experience as Certified Aide preferred.Functional DemandsPhysical RequirementsSitting - 16-31%Walking - 1-15%Standing - =>32%Bending/Squatting - =>32%Climbing/Kneeling - 1-15%Twisting - =>32%OSHA CategoryMinimal potential for direct exposures.Visual and Hearing RequirementsMust be able to see with corrective eye wear.Must be able to hear clearly with assistance.Other Physical/Environmental DemandsLifting - 0-50lbs, 50lbs or more with assistanceCarrying - 0-50lbs, 50lbs or more with assistancePulling - up to 100lbsPushing - up to 100lbsLeadership CapabilitiesMission, Service, and ValuesSupports the hospital Mission, Service and Values.Essential FunctionsProvides personal hygiene by method appropriate to patient's functional level and age group.Assists with transfers.Performs homemaking service including housekeeping, laundry, and shopping.Provides respite care.Performs wound care as directed by nurse.Completes charge sheets and visit documentation daily.Relays essential information from patient to aide supervisor.Back Up: Provides emotional support for terminal care patients and their families.Back Up: Participates in team meetings.Sets up and operates home medical equipment following specified parameters of treatment and maintains delivery logs.Educates patient and family on the safety and use of home medical equipment.Provides on call coverage for new set up/pickups and resolves equipment problems.Performs assessments to ensure home is safe for home medical equipment delivery, use, and enforces safety rules.Organizational ExpectationsT.J PledgeI Will Show Compassion-see all patients, family members, coworkers, physicians, and anyone else at T.J. as my customer.-recognize that each and every interaction I have at T.J.
