Transitions At Home is currently seeking to hire a compassionate and experienced Full‑Time RN Case Manager to join our growing home health team. What does a Transitions at Home RN Case Manager do? Registered Nurse Case Managers provide skilled nursing care including assessments, medications, treatments, and coordinate services. They counsel the client and their family or significant others in the client’s residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
