Hospice Registered Nurse (RN) – Home Health | Full-Time

We are seeking an experienced Hospice Registered Nurse (RN) to provide compassionate in-home hospice care throughout Washington, Racine, Kenosha, and Walworth counties in Wisconsin . This is a full-time, Monday–Friday, 8:00 AM–5:00 PM position with no on-call responsibilities , offering a consistent schedule and 40 guaranteed hours per week .

Position Highlights:

• Monday–Friday, 8:00 AM–5:00 PM
• No on-call or weekends
• 40 guaranteed hours per week
• Approximately 5 patient visits per day
• Visits only – no case management or IDG meetings
• Mileage reimbursement: $0.53/mile

Responsibilities:

• Provide skilled nursing care and comprehensive patient assessments in the home.
• Deliver hospice care while managing symptoms, wound care, and patient education.
• Collaborate with physicians and the interdisciplinary care team to ensure high-quality patient care.
• Document patient visits accurately using an electronic medical record (HomeCare HomeBase preferred).
• Advocate for patients and families while maintaining the highest standards of compassionate care.

Qualifications:

• Active Wisconsin RN license
• Previous hospice nursing experience required
• Wound care experience
• Current CPR/BLS certification
• Valid driver's license, reliable transportation, and current auto insurance
• HomeCare HomeBase (HCHB) experience preferred; MatrixCare or other EMR experience accepted

• Skills competency checklist
• Any additional clinical education completed within the past 12 months
