TravelNurseSource is working with Conflux Systems Inc to find a qualified Long-Term Care RN in Hilo, Hawaii, 96720!

Description

Long Term Care facility on site of hospital. Will be performing assessments, treatments, medication administration, notification to MD/NP of pertinent information, admitting/discharging, ADL assistance, Gastrostomy Tube care, Urinary Catheter care, Nebulized Treatment, Wound Vac care, CPAP care, Tracheostomy Care and other assigned duties.

Key Responsibilities

• Assess residents' physical, mental, and emotional health status.
• Develop and update individualized care plans.
• Administer medications, treatments, and intravenous therapies as prescribed.
• Monitor residents for changes in condition and report findings to physicians and healthcare teams.
• Maintain accurate medical records and documentation.
• Supervise and support Licensed Practical Nurses (LPNs), nursing assistants, and other care staff.
• Respond to emergencies and provide appropriate interventions.
• Educate residents and families about health conditions, treatments, and care plans.
• Coordinate care with physicians, therapists, dietitians, and other healthcare professionals.
• Ensure compliance with infection control, safety, and regulatory standards.

Requirements

Qualifications

• Valid Registered Nurse (RN) license.
• Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
• CPR/BLS certification (required).
• Experience in long-term care, geriatrics, rehabilitation, or skilled nursing preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal abilities.
