Senior Living Occupational Therapist- Home Health

Senior Living Occupational Therapist – Full Time Position located at BAYADA Home Health Care’s Vermont Senior Living office in Williston, Vermont. The role performs outpatient home‑health visits for adult and geriatric patients throughout Addison County, Vermont. As a home‑care Occupational Therapist, you will be an integral member of a multidisciplinary health‑care team that delivers rehabilitative care and skilled nursing to clients, enabling them to remain safely at home. Responsibilities Perform outpatient therapy evaluations to develop a plan of care for patients in senior‑living communities or group home settings for adults with intellectual disabilities. Operate at the top of licensure using current research‑based approaches to maximize functional independence for clients. Collaborate closely with home‑health therapy partners to transition care from home health to outpatient services, achieving client‑centered goals in the client’s living environment. Educate clients, families, and community caregivers on care plans to support a team approach for maximizing functional independence. Perform evaluations, treatments, and appropriate discharge planning utilizing an outpatient EMR platform to document individualized treatment approaches. Qualifications A current Vermont Occupational Therapist license. Prior home‑care experience preferred, but not required. At least one year of recent work experience as an Occupational Therapist. Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, evidenced by school transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills. Additional Information Base Pay, depending upon qualifications and subject to negotiation: $45‑50 per hour.
