Reliant Rehabilitation is seeking a licensed physical therapist to provide comprehensive therapy services across assisted living, independent living, and memory care communities. You will screen, evaluate, and treat residents, develop plans of care, discharge planning, and supervise PTAs and Rehab Techs. You will collaborate with the interdisciplinary team to promote patient-centered care while meeting OBRA, state, and regulatory requirements and ensuring high-quality outcomes. #J-18808-Ljbffr Reliant Rehabilitation
