Description

• Assess patients across inpatient and outpatient environments, crafting personalized treatment strategies
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Engage with nurses, physicians, case managers, and fellow rehab specialists to optimize patient outcomes
• Guide patients and families in understanding therapeutic objectives, discharge plans, and home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (neurological, geriatric) are advantageous but not essential

Skills

• Preferably seasoned experience in the field
• Capable of treating patients with complex medical and mobility challenges
