Registered Nurse - Home Health

SCHEDULE
Shift: 8 hours

ASSIGNMENT DETAILS
Contract Length: 12 weeks
Guaranteed Hours: 40

REQUIREMENTS
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Current CPR certification
- Current driver's license
- Reliable transportation and insurance

ROLES & RESPONSIBILITIES
- Administer skilled nursing care for clients of all ages in their place of residence.
- Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and/or their families regarding care, medications, and home safety.
- Supervise and provide clinical direction to nursing assistants to ensure quality service.
- Perform on-call responsibilities as assigned and provide effective communication among team members and healthcare professionals.
