This role is responsible for ensuring eligible patients receive timely breast cancer screening, coordinating diagnostic follow-up for abnormal findings, communicating results, and facilitating access to appropriate specialty care. The coordinator plays a critical role in reducing gaps in care, improving screening compliance, and supporting patients throughout the breast cancer screening and diagnostic process.Working within a complex, multidisciplinary care environment, the coordinator must demonstrate exceptional communication, organizational, and collaboration skills to ensure seamless coordination across multiple departments and providers. Success in this role requires the ability to proactively share information, clarify responsibilities, and maintain strong working relationships to prevent inefficiencies, duplication of efforts, delays in care, and communication breakdowns.In addition, this role performs all clerical duties required to complete patient appointment and procedure scheduling, manage incoming and outgoing phone communications, and perform other related administrative activities. The coordinator provides exceptional customer service through courteous, accurate, proactive communication and problem-solving to ensure patients receive timely and effective care.Under the supervision of the Clinical Lead and Practice Leader, the coordinator is also responsible for assisting in the delivery of healthcare and patient care management within their scope of practice. This includes gathering patient information, assessing patient requests, obtaining and monitoring vital signs, assisting with procedures, and reprocessing instruments.Job Requirements, Prerequisites and Essential Functions:Pay Range:$29.49 - $34.33 - $40.05Essential Functions and Responsibilities:Breast Cancer Screening CoordinationIdentify, track, and conduct outreach to patients due or overdue for breast cancer screening in accordance with established clinical guidelines.Coordinate and facilitate scheduling of screening mammograms and recommended breast imaging studies, including ultrasound, MRI, and diagnostic follow-up procedures.Maintain screening registries and tracking systems to monitor patient compliance, outcomes, and timely completion of care.Collaborate with primary care providers, OB/GYN providers, RN breast care navigators, and imaging departments across multiple locations to ensure seamless patient management and continuity of care.Diagnostic Follow-Up and Care NavigationMonitor and track breast imaging results, ensuring timely provider review, patient notification, and completion of recommended follow-up care.Coordinate diagnostic testing, biopsies, specialty referrals, surgical consultations, and other services required to support timely diagnosis and treatment.Identify and address barriers to care, escalating delays or concerns according to established protocols.Ensure patients with suspicious or confirmed breast cancer diagnoses are connected promptly to appropriate specialty services and resources.Maintain oversight of external records and results, ensuring documentation is received, reviewed, and incorporated into the electronic health record within established timeframes.Interdisciplinary Communication and Care CoordinationServe as a central point of coordination among patients, providers, breast imaging facilities, breast surgeons, oncology specialists, pathology departments, and ancillary services.Facilitate the timely exchange of orders, imaging reports, pathology findings, consultation notes, and other clinical information necessary to support patient care.Demonstrate exceptional communication and collaboration skills to ensure alignment among care team members, minimize delays, prevent duplication of efforts, and reduce gaps in communication.Participate in multidisciplinary discussions and care planning activities to support efficient, patient-centered care delivery.Patient Education and SupportEducate patients regarding breast cancer screening recommendations, diagnostic procedures, referrals, and treatment pathways.Provide compassionate guidance and support throughout the screening and diagnostic process, addressing questions, concerns, and anxiety related to testing and results.Assist patients in navigating healthcare systems and accessing appropriate resources to support timely care.Documentation, Quality Management, and Program SupportDocument patient interactions, care coordination activities, and follow-up efforts in the electronic health record (EHR).Maintain accurate records of screening completion, results communication, diagnostic follow-up, and referral outcomes.Participate in program-wide data tracking, quality reporting, and certification management activities to support breast health program goals and regulatory requirements.Attend multidisciplinary team meetings and collaborate on periodic compliance audits, quality improvement initiatives, and population health efforts focused on early detection and timely diagnosis of breast cancer.Ensure compliance with HIPAA and organizational privacy, confidentiality, and documentation standards.Additional Direct Patient Care & Administrative Support:Room patients and collect vital signs, medical history, and visit information.Deliver compassionate, patient-centered care and address patient questions and concerns.Administer injections, immunizations, and other treatments as directed by providers.Assist providers with examinations, procedures, and patient care.Triage patient needs and respond appropriately to urgent situations.Manage patient communications, including lab results, prescriptions, referrals, and follow-up care.Accurately document patient encounters and maintain electronic medical records (EMR).Coordinate referrals and support care continuity while ensuring patient confidentiality.Maintain exam rooms, clinical supplies, and a clean, organized care environment.Support daily clinic operations, including inventory management and administrative tasks.Answer and route incoming calls professionally and courteously.Schedule, coordinate, and confirm patient appointments.Greet, check in, and assist patients with requests, questions, and concerns.Verify insurance eligibility, obtain prior authorizations, and assist patients with insurance-related questions.Resolve routine patient issues while maintaining confidentiality and professionalism.Support departmental and organizational goals through effective teamwork and service.Qualifications:Education:High school diploma or General Educational Development (GED) certificate required.Completion of business school or related training, including computer courses preferred.Medical Assistant certification.Experience:At least one year of related medical assisting experience in a physician’s office or other clinical setting, or equivalent combination of training and experience.Re-processing/procedural experience is highly preferred.One (1) years of progressive responsibility and directly related work experience.Experience with, and proficiency in, electronic medical records: Use of EPIC software a plus.Computer proficiency: Microsoft Office.Bi-lingual in Spanish desired.Additional Preferred Qualifications:Minimum of 2 years of experience in a healthcare setting, preferably in women's health, primary care, radiology, oncology, breast health, or care coordination.Experience coordinating mammography, breast imaging, or oncology-related services.Familiarity with breast cancer screening guidelines and breast imaging workflows.Experience with
