Nurse Practitioner or Physician Assistant

Marin City Health and Wellness Center Family Nurse Practitioner/Physician AssistantThe Marin City Health and Wellness Center MCHWC Family Nurse Practitioner/Physician Assistant works collaboratively with the Chief Medical Officer to provide a wide range of primary care services which includes health promotion, injury and illness prevention, diagnosis, treatment, and monitoring of commonly encountered health problems and injuries. As a part of the MCHWC key staff, the mid-level practitioner works with the MCHWC's Chief Medical Officer to serve as a community liaison and advocate. The Practitioner upholds the standards and guidelines of the MCHWC and provides healthcare services in a holistic, patient care-centered framework.Essential Duties and Responsibilities:Assesses the health status of patients by reviewing medical histories, initiating, performing screening and diagnostic tests, and other procedures appropriate for the patient's condition.Assist the CMO in medical procedures.Prepare care plans for patients and monitor the conditions of the patients.Interpret and evaluate individuals' health care requirements.Write prescriptions.Communicate with patients closely and emphasize disease prevention and wellness strategies.Contribute to the success of the Center through quality improvement initiatives.Maintain professional working relationships with all staff, clients and the public.Knowledgeable of policies and procedures for safe work practices.Demonstrate knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care.Consult with CMO as dictated by patient need and assessment findings.Establish and maintain communication with other MCHWC providers, staff, and volunteers to promote consistency and continuity of care.Supervisory Responsibility: N/AQualification Requirements:Education and/or Experience:Current license to practice as a Nurse Practitioner or Physician Assistant in CaliforniaMaster's Degree in Nursing and National Certification from accredited schoolsExperience working in an outpatient health clinic setting. Experience in a community health center setting working with underserved populations preferred.Valid DEA CertificateCurrent CPR (BLS) certifications.Experience working in an outpatient health clinic or office setting. Experience in a community health center settingExperience with administration of community health programs, and staff supervisionDemonstrated experience with Electronic Medical Records.Mission-driven and passionate about community health issues.Language Skill:Excellent customer service and communication skillsStrong verbal, written, and interpersonal skillsReasoning ability:Ability to recognize problems, collect data, and establish factsAbility to understand and apply guidelines, policies and proceduresEquipment/Machinery: Fax machine, copier, personal computer, telephone, calculator, Microsoft Word/Excel, EHR system and other software as required.PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an individual to successfully perform the essential functions of this job.
