As a Health Home Care Manager (HHCM) at JCCA, you'll play a vital role in helping children and families navigate complex medical, behavioral, and social service systems.Using a strength-based, person-centered approach, you'll coordinate care, build individualized plans, and ensure families receive the support they deserve—when and where they need it.The Health Home Care Manager (HHCM) is required to have a detailed understanding of the health focused work and is primarily responsible for assessments, service plan development, case coordination, linking and advocacy; monitoring, and clinical consultation. Their focus is on coordinating and arranging for the provision of services; supporting adherence to treatment recommendations; and monitoring and evaluating a member's needs, including prevention, wellness, medical, specialist and behavioral health treatment, care transitions, and social and community services where appropriate through the creation of an individual plan of care. The HHCM is expected to use an individualized care service delivery approach ensuring the services are designed to address the unique needs and strengths of each member.Key Responsibilities:Conduct face-to-face assessments and create individualized Plans of Care (POC)Facilitate communication across medical, behavioral, and social service providersHelp members understand treatment plans and improve health literacyParticipate in care planning meetings, team supervision, and ongoing trainingEnsure timely and accurate documentation in Health Home IT platformsSupport foster care cases with monthly collaboration with case planning teamsEngage members through outreach—phone, mail, electronic, and in-personStay up-to-date with Medicaid Managed Care, SPA, and HCBS guidelinesQualifications:Bachelor's Degree (BA or BS) with 2+ years of relevant experienceWorking with children and families with special needs, disabilities, or complex traumaExperience in client engagement & outreach and person-centered planningCoordinating care and linking families to community-based servicesStrong interpersonal and communication skillsAbility to work collaboratively in a fast-paced, team-oriented environmentFlexible and responsive to changing program needsOrganized and tech-savvy—comfortable with electronic documentation platformsWillingness to travel via public transit for home visits and meetingsRequired Certification/Licensing:Mandated ReportingMedicaid CANS certification and on-going recertificationHourly Pay Rate: $27.47 - $29.71 - differential offered for Spanish or Creole language skills
