Care Coordinator

FirstLight Home Care | Serving Frisco and Plano Great home care takes someone who knows the clients, supports the caregivers, and follows through. We're looking for an experienced Care Coordinator who combines compassion with strong organizational skills and the confidence to lead. You'll help families navigate care, build dependable caregiver relationships, and keep daily operations running smoothly. This position requires regular in-office attendance, local travel for client visits, and participation in an every-other-week on-call rotation. What you'll do Build trust with clients and families. Serve as a primary point of contact, communicate proactively, address concerns, and follow through on commitments. Coordinate care from the start. Conduct initial and ongoing in-home care assessments, document client needs and preferences, introduce caregivers, and coordinate services. Keep care dependable. Manage schedules, address call-offs, and match caregivers to clients based on care needs, skills, availability, and compatibility. Stay ahead of problems. Review upcoming schedules, identify coverage gaps, monitor overtime, and arrange appropriate coverage before issues affect clients. Lead and support caregivers. Participate in recruiting, onboarding, training, coaching, and performance management. Set clear expectations and hold caregivers accountable with fairness and respect. Monitor service quality. Conduct client visits and caregiver check-ins, respond to changes in needs, and escalates concerns promptly. Keep WellSky accurate and current. Maintain schedules, client and caregiver records, care notes, and communication documentation. Support agency growth. Help new clients transition smoothly into services and build relationships that encourage retention and referrals. Share after-hours responsibility. Be on call every other week to respond to urgent scheduling and client-care needs. What you'll bring At least one year of hands-on WellSky Personal Care experience-required. One to three years of home care or healthcare experience, preferably coordinating nonmedical home care. Previous scheduling experience, including managing last-minute changes and competing priorities. Management or supervisory experience, with the ability to coach employees and address performance concerns directly. Strong communication skills, sound judgment, and attention to detail. The ability to remain calm when a caregiver calls off, a family needs answers, and priorities change. Reliable follow-through: you document decisions, communicate updates, and make sure issues are resolved. Availability to work in the office, conduct local client visits, and fulfill the every-other-week on-call rotation. What success looks like Families feel informed and supported.
