Home Health Registered Nurse Case Manager
EHS Home Health Care Services, Inc.
EHS Home Health Care Services, Inc., part of Advocate Health, is seeking a compassionate Home Health RN Case Manager to serve patients in Evanston, Skokie, Lincolnwood, and Morton Grove. Provide skilled nursing in patients’ homes as they recover from illness or surgery or manage chronic conditions. Assess patient needs, develop and update care plans, monitor progress, educate patients and caregivers, and coordinate services with physicians and an interdisciplinary team. Accurate clinical documentation and a commitment to safe, high-quality care are essential. Join a respectful, supportive, patient-centered team with access to clinical resources, training, and career growth across a respected health network. Illinois RN licensure is required; home health or case management experience is preferred.
