As a Nurse Practitioner with Carease Health, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities.

About the Role:

As a hybrid Nurse Practitioner, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role will depend on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities:

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team
• Collaborate with healthcare teams to develop and implement individualized care plans
• Maintain accurate and thorough documentation of patient care
• Participate in quality improvement initiatives to minimize hospitalizations and improve patient outcomes

Requirements:

• Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
