We are seeking a dedicated and compassionate Part-Time or PRN Physical Therapist to join our team at Green Palms Health & Rehab at Crawford in Cleveland, OH. As a valued member of our interdisciplinary team, you will play a crucial role in providing exceptional rehabilitation services to our residents.

About the Role:

As a Physical Therapist with our organization, you will have the opportunity to work with a diverse patient population, ranging from short-term rehabilitation to long-term skilled nursing care. You will be responsible for developing and implementing personalized treatment plans to promote optimal physical function, strength, and mobility. Your expertise will enable our residents to regain their independence, confidence, and overall well-being.

Responsibilities:

• Conduct thorough initial and ongoing assessments to identify physical needs and develop individualized treatment plans

• Plan and implement evidence-based physical therapy interventions to promote functional mobility, strength, and range of motion

• Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal patient outcomes

• Maintain accurate and detailed records of patient progress and treatment plans

• Participate in interdisciplinary team meetings to discuss patient care and make recommendations for updates to treatment plans

Requirements:

• Current Physical Therapy license in the state of Ohio

• Minimum of 0 years of experience in a healthcare setting (new graduates welcome to apply!)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Commitment to providing exceptional patient care and support
