**Exciting Opportunity for a Case Manager - Registered Nurse** **Job Description:** - Join a dynamic team as a Case Manager, where you will play a vital role in patient care and resource management! - Shift: 5x8 hour days (08:30 - 17:00) - Block schedules are available for a balanced work-life experience. - Bring your expertise with at least 2 years of experience preferred in case management. - **Responsibilities:** - Conduct medical necessity reviews, ensuring quality patient care. - Master discharge planning to facilitate smooth transitions for patients. - Engage in utilization review, collaborating with the healthcare team to optimize patient outcomes. - Identify and communicate any changes in the patient’s plan of care effectively. - Maintain comprehensive knowledge of community resources for effective referrals. - Leverage length of stay (LOS) data to anticipate patient needs and resource management. - Document meticulously in patient records, including initial screenings and discharge plans to ensure continuity of care. - **Patient Ratios:** Enjoy manageable ratios with 1:20 on days. - **Orientation:** Receive 24 hours of orientation to acclimate smoothly. - **Patient Age Groups:** Work with a diverse range of patients from adults to pediatrics and geriatrics. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
