Physical Therapist Job Summary Oasis at the Keys Nursing and Rehab is seeking a dedicated and compassionate Physical Therapist to join our team in Tavernier, Florida. As a Physical Therapist at Oasis at the Keys, you will have the opportunity to provide exceptional patient care and contribute to the rehabilitation journey of our residents. Responsibilities Assess referred patients that require rehabilitation services and develop personalized treatment plans Provide direct patient care, including evaluations, treatments, and education Establish and administer treatment programs with specific goals determined according to patients' capacity and tolerance Document and record patients' conditions, progress, and treatment plans Collaborate with other disciplines to provide patient‑centered care Meet all regulatory requirements, including evaluations, weekly progress notes, recertification, and discharge documentations Perform all other duties assigned by the Director of Rehabilitation Requirements/Qualifications Graduation from an accredited Physical Therapy college or university Valid professional license and registration for the state of practice Experience with long‑term care and/or geriatric care is preferred, but not limited to Experience with PPS scheduling is preferred, but not limited to Join our team and experience the Oasis at the Keys difference! #J-18808-Ljbffr Oasis at the Keys Nursing and Rehab
