Home Health Aide – Make a Difference in Your Community

About the Role:

As a Home Health Aide with Modani Care, you will provide direct patient care, assist with daily living activities, and promote independence and well-being for our clients. Your compassionate and dedicated approach will help us deliver exceptional care that exceeds our patients' expectations.

Responsibilities:

• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Provide medication reminders and assistance with medication management
• Help with meal preparation and light housekeeping
• Assist with transferring patients to and from wheelchairs or beds
• Monitor patients' conditions and report changes to our healthcare team
• Maintain accurate and detailed records of patient care and activities
