Case Management Nurse

We're seeking a compassionate and experienced Registered Nurse (RN) Coordinator to join a busy outpatient team. In this role, you'll coordinate patient care, collaborate with providers and clinical staff, and help ensure patients receive high-quality, seamless care throughout their healthcare journey. Responsibilities Coordinate patient care plans across providers, departments, and ancillary services. Perform clinical triage via phone, patient portal (EMR), and in person. Review laboratory, imaging, and diagnostic results, escalating concerns as appropriate. Prepare patients for procedures and ensure all pre-operative requirements are completed. Educate patients on treatment plans, medications, and follow-up care. Coordinate referrals, prior authorizations, and specialty appointments. Accurately document all patient interactions in the electronic medical record (Epic). Serve as the primary point of contact for patients, families, providers, and the care team. Monitor patient progress and identify opportunities to close gaps in care. Support quality improvement initiatives and departmental goals. Qualifications Active Registered Nurse (RN) license required. 2-3+ years of clinical RN experience required. 3-5+ years of nursing experience preferred. Family Medicine or outpatient clinic experience strongly preferred. Care coordination or case management experience preferred. Experience with clinical triage and patient assessment. Proficiency with Epic or a comparable electronic medical record system. Strong patient education, communication, and organizational skills. Ability to manage multiple priorities in a fast-paced, team-oriented environment.
