COMPENSATION & BONUS STRUCTURE

$250,000 Base Salary – New Residents
$295,000 Base Salary – Experienced Physicians
Annual Incentive Bonuses: $45K–$85K+
Bonus potential up to 28% of base salary
Potential Total Compensation: $350K–$380K+ annually

PRIMARY RESPONSIBILITIES
━━━━━━━━━━━━━━━━━━━

Evaluate and treat patients according to established clinical standards
Coordinate specialty referrals, hospital care, SNF coordination, home health services, and durable medical equipment needs
Collaborate with the care team through daily huddles and patient care discussions
Maintain timely and accurate EMR documentation
Participate in quality improvement initiatives and outcome reviews
Deliver patient-centered care within a value-based healthcare model
Foster a positive, professional, and team-oriented environment

REQUIRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━

✔️ MD or DO from an accredited medical school
✔️ Active and unrestricted medical license (or willingness to obtain)
✔️ Board Certified or Board Eligible in:
• Family Medicine
• Internal Medicine
• Geriatric Medicine

✔️ Active DEA License
✔️ Medicare & Medicaid Provider eligibility
✔️ Strong communication and interpersonal skills
✔️ Comfortable in a collaborative team-based environment
✔️ Basic EMR/EHR and computer skills

⭐ PREFERRED EXPERIENCE

✅ Experience with Medicare Advantage patients
✅ Knowledge of HEDIS/STARS metrics
✅ Familiarity with CAHPS & HOS measures
✅ Experience serving culturally diverse and underserved populations
✅ Prior value-based care experience preferred
✅ EMR/EHR experience
