COMPENSATION & INCENTIVES

$250,000 Base Salary – New Residents
$295,000 Base Salary – Experienced Physicians
Annual Performance Bonuses: $45K–$85K+
Incentives can reach up to 28% of annual base salary
Potential Total Earnings: $350K–$380K+ annually

KEY RESPONSIBILITIES
━━━━━━━━━━━━━━━━━━━

Evaluate and manage patients according to evidence-based clinical standards
Coordinate specialty referrals, hospital care, SNF coordination, home healthcare, and durable medical equipment needs
Participate in daily clinical team huddles and collaborative care discussions
Complete timely and accurate EMR documentation
Review quality metrics and patient outcomes with clinical leadership
Deliver compassionate, patient-focused primary care
Support a positive and team-oriented practice environment

REQUIRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━

✔️ MD or DO from an accredited medical school
✔️ Active and unrestricted medical license (or willingness to obtain)
✔️ Board Certified or Board Eligible in:
• Family Medicine
• Internal Medicine
• Geriatric Medicine

✔️ Active DEA License
✔️ Medicare & Medicaid Provider eligibility
✔️ Excellent communication and interpersonal skills
✔️ Comfortable working within a collaborative team-based model
✔️ Basic EMR/EHR and computer proficiency

⭐ PREFERRED EXPERIENCE

✅ Medicare Advantage experience
✅ Knowledge of HEDIS/STARS metrics
✅ Familiarity with CAHPS & HOS
✅ Experience working in underserved and culturally diverse communities
✅ Prior value-based care experience preferred
✅ Electronic Medical Record (EMR/EHR) experience
