Position Physical Therapist (PT) – Rockville territories, Adventist HealthCare Home Health Responsibilities Perform an initial examination and evaluation to establish an impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings (flexes to any Rehab site as needed). Provide or direct and supervise therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care, involving the patient/client and appropriate others in planning, implementation, and assessment of care. Reexamine the patient/client as necessary during an episode of care to evaluate progress or change in status and modify the plan accordingly or discontinue services. Plan for discharge of the patient/client considering achievement of anticipated goals and expected outcomes, and provide appropriate follow-up or referral. Communicate, coordinate and document all aspects of patient/client management including examination results, diagnosis, plan of care, interventions, responses, barriers, status changes, reexamination, discharge/discontinuation, and other management activities with members of the care team as indicated. Supervise and direct the patient care activities of assigned rehab aides, office assistants, and volunteers. Manage schedule to accommodate range in census and patient load and inform management of any issues with caseload management. Attend team meetings and effectively communicate patient status, using the medical record or other treating therapists to ensure sufficient knowledge of the case. Be responsible for individual professional development activities. Qualifications Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program. 1-year site‑specific preferred. Active Maryland license. Valid driver’s license with agency insurance requirements. Active American Heart Association Basic Life Support (BLS) certification required. Work Schedule Employment Type: Part‑time. Hours per Week: 24 hrs/week. Typical Daily Schedule: 8:00 AM – 5:00 PM. Every Monday, Wednesday, and Friday. Shift Type: Days. Weekend Requirements: One weekend per month. Holiday Requirements: Rotating. Pay Range $34.58 - $51.87 per hour.
