Position Overview We are seeking a Hospice Social Worker to provide emotional, psychosocial, and practical support to patients and families. The Social Worker plays a key role in the interdisciplinary team and helps ensure patients receive holistic, patient-centered care. Key Responsibilities Conduct comprehensive psychosocial assessments Provide counseling and emotional support to patients and families Assist with advance directives and care planning Coordinate community resources and support services Participate in interdisciplinary team (IDT) meetings Maintain accurate and timely documentation Ensure compliance with hospice regulations and standards Qualifications Licensed Social Worker in Massachusetts (LCSW or LMSW) Previous hospice or healthcare experience preferred Strong communication and interpersonal skills Ability to work independently and as part of a team Valid driver's license and reliable transportation Benefits Competitive compensation Health, dental, and vision insurance Paid time off Supportive team environment Opportunities for growth within a developing organization #J-18808-Ljbffr Solara Hospice
