Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Address, identify, and reassess cost-efficient, appropriate levels of care.
• Facilitate non-medical support to ensure treatment compliance, such as housing and transportation.
• Coordinate care transitions as needed.
• Engage with physicians, social workers, discharge planners, and claims professionals.
• Utilize specialty resources to achieve optimal resolutions.
• Maintain detailed records of outcomes during the management process.
• Identify opportunities for health promotion and illness prevention.
• Act as a patient advocate by safeguarding privacy and confidentiality.

Requirements of the RN - Case Management - Travel

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license (compact license accepted).
• Ability to analyze complex medical information and make rational decisions.
• Case Management certification.
