Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for comprehensive care plan implementation
• Identify and reassess cost-efficient, appropriate levels of care
• Ensure compliance with treatment plans through non-medical support
• Coordinate care transitions as needed
• Coordinate with healthcare professionals during patient care transitions
• Engage specialty resources for optimal resolution
• Maintain accurate records of clinical, functional, and fiscal outcomes
• Identify health promotion and illness prevention opportunities
• Act as a patient advocate protecting privacy and confidentiality

Requirements of the RN - Case Management - Travel

• RN licensure (can accept compact license)
• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Case Management certification
• Home Health care experience, HCHB, OASIS required
• Med surg/critical care skills preferred
