Physical Therapist – Home Health (Madison County, PRN) Sign‑on Bonus: $10,000 Special consideration for applicants who currently live in or are commutable to Madison County. Our compensation plan offers top pay in the industry through a new pay‑per‑point model that rewards higher effort visits. Job Purpose Provide evaluative, consultative and treatment interventions to all home health patients in their place of residence referred by a licensed physician, while maintaining effective communication with home health partners, patients, families and other internal and external customers. Key Responsibilities Evaluate the patient’s level of function and skilled therapy needs. Treat the patient to relieve pain, develop or restore function, and achieve maximum outcomes according to the established plan of care. Advise and consult the family and other interdisciplinary team members to promote optimal functional outcomes. Assess, monitor, document and report progress of patient’s health and condition using required documents via clinician technology, within established timelines. Maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan, including referrals to other disciplines as needed. Demonstrate intermediate knowledge of home care and competency in discipline‑specific patient care skills required for the provision of care. Conduct field visits to initiate care and monitor clinical outcomes, follow‑up and care performance improvement initiatives. Utilize accepted company teaching materials in patient/family education. Complete documentation timely, accurately, and at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination). Meet agency productivity standards and use time and resources effectively. Participate in center/agency surveys (Licensure/Joint Commission), mandatory in‑services, case conferences, staff meetings, and programs such as the Performance Improvement Program. Follow established safety regulations and procedures, reporting any occupational hazards. Maintain current license, certification, and clinical records to meet federal, state, and agency regulations and guidelines. Recognize and respond to priorities in patient care, showing initiative and sound judgment. Demonstrate willingness to adapt to workload changes and assist with training/orienting others. Accept responsibility to report any alleged or suspected violations to supervisor and/or others until resolution. Be self‑directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines. Licensure, Certification, and Education Requirements Valid professional license in the state of practice. Graduate of an accredited Physical Therapy Program. Foreign‑trained candidates must have a program deemed substantially equivalent by an APTA‑approved credentials evaluation organization. Preferred Skills and Experience Prior home health experience is preferable.
