Position Summary

At JFS At Home, we are committed to helping older adults maintain their quality of life with safety, dignity, and independence in the comfort of their own homes. The Registered Nurse (RN) – Clinical Services & Community Engagement plays a vital role in advancing this mission through a unique combination of clinical leadership, caregiver support, client advocacy, community education, and relationship development.

Reporting directly to the CEO/Administrator, this position serves as both a clinical leader and community ambassador for the agency. While maintaining primary responsibility for client assessments, care planning, caregiver supervision, quality assurance, and clinical oversight, this position also develops and delivers educational programs, promotes companion services, supports long-term care insurance education initiatives, and cultivates relationships with healthcare professionals, referral partners, and organizations serving older adults throughout South Palm Beach County.

This position is designed to support JFS At Home's long-term strategic objective of increasing community awareness, strengthening referral relationships, supporting sustainable private-pay growth, and helping older adults age safely and independently in their homes.

   Primary Clinical Responsibilities

Client Assessments

• Conduct comprehensive initial client assessments.
• Perform reassessments as required by agency policy and client needs.
• Evaluate client care needs and recommend appropriate services.
• Identify changing conditions and communicate recommendations to clients, families, caregivers, and healthcare providers.

Care Planning

• Develop individualized plans of care.
• Review and update plans of care as client needs change.
• Coordinate care recommendations with clients, families, physicians, and caregivers.
• Ensure plans of care reflect the client's goals, preferences, and safety needs.

Caregiver Supervision

• Conduct supervisory visits in accordance with agency standards.
• Provide coaching, guidance, and support to caregivers.
• Monitor caregiver performance and client satisfaction.
• Assist with caregiver training and competency validation when needed.

Quality Assurance and Compliance

• Ensure compliance with agency policies and procedures.
• Maintain timely and accurate documentation.
• Participate in quality improvement initiatives.
• Assist in maintaining compliance with all applicable state and regulatory requirements.

Client and Family Support

• Serve as a resource to clients and families.
• Educate clients regarding care options and available services.
• Assist families in navigating changes in care needs.
• Promote client satisfaction and positive outcomes.

Companion Services Growth Initiative

Support agency efforts to increase awareness and utilization of companion services.

Responsibilities include:

• Educating seniors and families regarding companion services.
• Promoting social engagement and community connection.
• Helping families understand the benefits of early support services.
• Identifying opportunities for clients to begin services earlier in their aging journey.

Long-Term Care Insurance Education Initiative

Support JFS At Home's efforts to educate families regarding:

• Long-term care insurance benefits.
• Policy utilization strategies.
• Aging in place planning.
• Appropriate use of home care services.

Assist with community education events and informational consultations regarding long-term care planning.

Qualifications

Required

• Current unrestricted Florida Registered Nurse (RN) license.
• Minimum of three years of nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Excellent verbal and written communication skills.
• Ability to work independently and manage multiple priorities.

Preferred

• Home Health experience.
• Private Duty experience.
• Geriatric care experience.
• Dementia care experience.
• Community education or public speaking experience.
• Experience working with older adults and family caregivers.

Position Statement

This position enhancement recognizes the critical role nursing leadership plays in both client care and community engagement. Community education, outreach, and referral development responsibilities are intended to complement—not replace—the employee's clinical responsibilities. The position remains first and foremost a nursing leadership role dedicated to ensuring exceptional client care, while also supporting JFS At Home's mission, community presence, and long-term sustainability.
