Therapist - Physical Therapist

Job Description

Job Summary:

Provide skilled physical therapy services to patients within a skilled nursing facility, supporting recovery and functional improvement through tailored treatment plans.

Responsibilities:

• Assess patients' physical conditions and develop individualized therapy programs.
• Implement therapeutic interventions to improve mobility, strength, and coordination.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and staff on therapy techniques and injury prevention strategies.
• Document therapy sessions, patient responses, and progress notes accurately.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience in a skilled nursing or rehabilitation setting.
• Knowledge of therapeutic modalities and patient assessment techniques.
• Strong communication and clinical skills.
• Ability to work effectively within a team environment.
• BLS certification preferred.
