Responsibilities
- Conduct timely PT evaluations and develop individualized treatment plans for residents
- Provide evidence-based therapeutic interventions to improve mobility, gait, strength, balance, and activities of daily living
- Implement treatment plans, monitor progress, and adjust goals as needed
- Document patient progress, plan of care, and functional outcomes in alignment with facility and payer requirements
- Collaborate with an interdisciplinary team to support overall resident care and safety
- Ensure disk and equipment maintenance, safety, and infection control during therapy sessions

Requirements
- Current physical therapy license to practice in the United States (NH licensure or eligibility)
- Experience in a skilled nursing facility or rehabilitation setting preferred
- Ability to work 08:00-16:30 with guaranteed hours of 35 per week
- Willingness to commit to a 13-week travel assignment
- Strong communication and documentation skills
- Demonstrated professionalism and patient-centered care mindset
