JOB TITLE: REGISTERED NURSE/LICENSED PRACTICAL NURSE
DEPARTMENT: GIBSON CITY CLINIC, 222 N SANGAMON AVE, GIBSON CITY, IL 60936
GENERAL SUMMARY
This is a position for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in the State of Illinois required to provide professional nursing care to patients and assist medical providers in a clinic based setting with ability to perform administrative and certain clinical duties under the direction and supervision of appointed medical provider(s).

PRINCIPAL DUTIES AND RESPONSIBILITIES
1. Confidentiality is to be maintained at all times.
2. Prepares treatment rooms for patient examinations, keeping room neat, clean, orderly, and proper equipment and supplies readily accessible for type of visit and or procedure.
3. Ensures all locks on cabinets in exam rooms, lab area, sample storage, and vaccination fridge/freezer are unlocked at beginning of office hours and are locked after office closing hours and after last patient has left.
4. Courteously greets all patients and visitors.
5. Assesses patient's general condition and takes vital signs and weight.
6. Assists providers with examinations, diagnostic procedures, and treatments.
7. Provides basic patient care as ordered.
8. Records all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the patient's electronic medical record or OB paper chart.
9. Assists patients as needed with walking, walking devices, wheelchairs, transfers, undressing instructions, collecting specimens, preparing for exam, etc.
10. Interviews patients to obtain medical information.
11. Identifies patient learning needs and provides patient education materials, communicating physician advice/instructions as appropriate.
12. Adheres to infection control/safety guidelines keeping exam and work areas neat, clean, and in orderly fashion.
13. Knowledge of proper use of medical equipment including stethoscope, EKG machine, NST machine, ear washing equipment, speculums, scales, blood pressure cuff, and other equipment.
14. Refills prescriptions as guided by the provider and/or mid-level provider.
15. Demonstrates applied knowledge in areas of patient care resources/references, protocols, pertinent drugs, standards of care, and significant lab values.
16. Must demonstrate current competencies applicable to job position.
17. Contacts medical facilities or departments to schedule patients for tests and/or admission and/or completes necessary forms requested.
18. Schedules appointments for patients including office, referrals, further testing, etc.
19. Helps answer incoming and outgoing patient telephone calls, triage or page calls, reply or forward electronic patient messages ensuring to record all final outcomes of patient calls into electronic patient chart.
20. Assists office coordinator/receptionist as needed by answering calls, scheduling appointments, greeting patients, opening/closing office for operation following office protocol, turning phone message on or off for phone operation, etc. as needed.
21. Checks over provider's appointment schedules for organizing patient flow.
22. Performs routine laboratory tests, venipunctures, and sample analyses (after receiving proper training from GAH for sign-off)
23. Stocks and checks inventory for medical and lab supplies and reports items for reorder to office coordinator/C.M.A./manager.
24. May assist provider(s) with examination, treatment, handing of instruments and materials and various procedures as needed.
25. Confirms drug refills and or provides prescription information authorized by provider to pharmacies as well as call, print, and fax script(s) to pharmacies as needed.
26. Reviews provider's dash in electronic medical record software program and helps obtain medical records from outside facilities in patient files for respective provider on file for patient.
27. Obtains x-ray reports, emergency room notes, hospital notes, etc. as needed for follow-up visits for review by provider before rooming patient etc.
28. Completes various patient forms & requisitions as needed.
29. Sets up medical laboratory equipment and supplies as needed for blood draw(s) and or analysis.
30. Gathers spoiled laundry bag when receptacle is full for pickup. Laundry is picked up either Tuesday or Wednesday. If full call GAH Laundry @ ext. 2672 for pick-up.
33. Pre-soaks, cleans, dries, and packages surgical instruments according to proper protocol and places in appropriate container to be sent for sterilization thru GAH courier ensuring to log in sterilization binder.
34. Collects blood, tissue, or other laboratory specimens, completes order requisitions and/or other forms for submitting specimens, logs the specimens, and prepares them for testing and/or sending with GAH lab courier or outside lab. Outside lab pickups may requiring requesting from UPS/FedEx or other carrier.
35. Explains treatment procedures, medications, diets, and provider's instructions to patients.
36. Ensures all supplies (samples, medication, lab, other medical supplies) are dated and are in range of date and disposes of outdated supplies according to waste disposal protocols. (Only employees who are trained and certified in biohazard waste removal are to access biohazard closet.)
37. Administers injections, treatments, and medications.
38. Interprets and evaluates diagnostic tests to identify and assess patient's condition.
39. Engages in research activities related to medical office and nursing and shares new information learned to providers and staff.
40. Responsible for following through on delegated duties.
41. Respects & promotes patient rights.
42. Responds appropriately to emergency codes; alerting appropriate staff along with immediate supervisors immediately.
43. Other various duties as assigned by the physician/mid-level practitioner which are subject to change at any given time.
44. Discards all trash in regular trash receptacles including exam rooms, basement, waiting room, office areas, lab, etc. using proper personal protection equipment when handling at end of each office day or if needed before if full.
45. Removes filled biohazard bags as well as removes any full sharps containers to biohazard closet if certified in biohazard waste removal and replaces with new red bags or new approved sharps containers.
46. Prepares and submits annual Vaccines For Children provider application for renewal of program.
47. Maintains logs of each injection and administration of medication and immunizations given and keeps logs for minimum of 3 years.
48. Maintains logs of all vaccines present at clinic including expirations dates, lot numbers, and number of injections given per vaccine for vaccines for children (VFC) stock and commercial stock.
49. Listed as one of the primary contacts for emergency plan and temperature excursion alerts if vaccination refrigerator/freezer temperature(s) falls out of specified range.
50. Orders vaccinations for influenza, pneumococcal, office stock and VFC provided stock of immunizations as needed in a timely fashion.
51. Checks expirations of vaccines, samples, injectables, and medications to ensure none are expired.
52. Orders medications for patients (ex. Mirena/Nexplanon) as well as completes forms for prior authorization for medications and patient assistance programs.
53. Submits requests to GAH prior auth team for hospital imaging and other testing needing performed prior to scheduling patient.
54. Acts as a liaison between patient and provider.
55. Ensures laboratory logs are completed and kept in secure cabinet.
PHYSICAL REQUIREMENTS
1. Has the usual sitting, standing, and stooping of regular office jobs.
2. Must be able to use appropriate body mechanics techniques when making necessary patient transfers and helping patients with walking, dressing, etc. including lifting, bending, twisting, turning.
3. Requires manual dexterity to operate a computer, calculator, copy/fax machine, and other office and medical equipment.
4. Mobility to climb two flights of stairs carrying up to 50 pounds routinely. Must climb two flights of stairs minimum of twice daily for access to supplies and biohazard closet.
5. Frequent vigorous walking for daily functions of walking patient to care areas in a fast-paced office environment.
6. Basic prolonged standing during daily functions at nursing workstations.
7. Involves frequent contact with staff and patients and may require dealing with angry or potentially violent people.
8. Auditory sharpness to hear patient's verbal communication, gather data to relay.
9. Visual insights to see details at close range (within a few feet of the observer) in order to observe patient vitals, assist patients to appropriate areas, to perform various tests and services, and watch out for potential dangers.
10. Clarity of speech for others to be able to understand instructions and interpretation given.
11. Ability to tell when something is wrong or is like to go wrong.
12. Emotional stability conductive to dealing with patients and family members.
13. May be required to work beyond regular clinic hours.
14. Exposure to communicable diseases and other conditions common to clinic setting.
15. Will work in an office with co-workers where traffic may be constant, subjecting you to several work interruptions, which can produce stress and fatigue.
16. Physical strength to perform the following lifting tasks:
• Floor to waist - 40 pounds
• 14" to waist - 50 pounds
• Waist to shoulder - 20 pounds
• Shoulder to overhead - 10 pounds
• Carry 40 pounds for 30 feet
• Push 40 pounds/force for 30 feet
• Pull 40 pounds/force for 10 feet REPORTING RELATIONSHIP
Reports to Office Coordinator/Receptionist, Mid-Level Provider, Respective Physician, GAH Administration EDUCATION, KNOWLEDGE AND ABILITIES REQUIRED
1. Must be licensed in the State of Illinois as a Registered Nurse or a Licensed Practice Nurse and certification must be kept current.
2. Knowledge of medical terminology; medical coding including ICD-9, CPT, & HCPC codes.
3. Minimum of one year work experience in health care. Knowledge of health care field and medical specialty
4. Proficient in mathematics for injectables, vaccinations, vital signs, weights, heights, medications, various tests, etc.
5. Ability to recognize, evaluate and solve problems and correct errors.
6. Good communication skills including the ability to speak clearly and concisely to assist patients.
7. Ability to project a pleasant and professional image.
8. Ability to learn and retain information regarding patient care procedures.
9. Ability to plan, prioritize and complete delegated tasks.
10. Ability to demonstrate compassion and caring in dealing with others.
11. Bilingual in English and Spanish suggested.
12. CPR/AED certificate is required according to the Rural Health Clinic Association.
13. Ability to type 50wpm (sixty words per minute) or more.
14. Ability to utilize a variety of computer software applications.
15. Trained and/or certified in phlebotomy to perform blood draws.
INFECTION EXPOSURE RISK LEVEL
Category 1-Maximal Risk-this job exposes you to potential blood, body fluids and tissues on a routine basis. It is recommended that you get the Hepatitis B vaccine. Yearly flu vaccination and tuberculosis testing are required.
WORKING CONDITIONS
1. Works in patient care areas where there are few discomforts due to dust, dirt, noise and the like.
2. Works with patients and may be exposed to contagious diseases or infectious materials, but potential for personal harm and injury is limited when proper safety and health precautions and equipment are used.
