Responsibilities:

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists to ensure optimal patient outcomes.
• Designs and implements individualized care plans based on thorough assessment and incorporates participation by significant others when appropriate.
• Conducts therapy practice within defined standards of care and directs support staff to achieve patients functional goals.
• Documents evaluations, treatment goals, and plans while maintaining complete and updated prescriptions and signatures.
• Promotes Elara Carings philosophy and acts as a resource to promote physical therapy in the community.
• Requires handling, lifting and transfer of patients; may work irregular hours.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved program.
• Current, unrestricted Physical Therapy license valid in the state of practice.
• Minimum one year of experience in a healthcare facility.
• Ability and willingness to travel within branch coverage area (up to 50%).
• Must have a dependable vehicle, valid drivers license, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.
