About the Role:

Responsibilities:

• Conduct comprehensive assessments to evaluate patients' physical abilities and needs.
• Develop personalized treatment plans aimed at achieving individual rehabilitation goals.
• Implement therapeutic exercises and modalities to enhance mobility and reduce pain.
• Educate patients and families on treatment strategies and home exercise programs.
• Monitor progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Collaborate with other healthcare professionals to provide integrated care.
• Maintain accurate documentation and records of patient progress and treatment.
• Stay updated on the latest therapeutic techniques and industry best practices.

Requirements:

• Valid state license to practice as a Physical Therapist in Connecticut.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Experience in home health care or outpatient settings is preferred.
• Strong communication and interpersonal skills to engage with patients and families.
• Ability to work independently and as part of a collaborative team.
• Compassionate demeanor with a commitment to patient-centered care.
• Knowledge of current evidence-based practices in physical therapy.
• CPR and First Aid certification is a plus.
